A gentle reminder that the coming Annual General Meeting of USRC is to be held on June 21, 2016 (Tuesday).
Kindly note that any Voting Member (or spouse) of the USRC can appoint another Voting Member as Proxy to vote if he/she cannot personally attend the meeting.
Proxy forms are now available at the Reception of the Club. It is also attached herewith for your easy reference. You can print it out (total 2 pages) and fill in the necessary information. For corporation members, please read note 4 on page 2 to ensure the validity of the proxy.
1) If you intend to have the LBS Subcommittee to vote on your behalf, please fill in either
YEUNG SHOU FONG ANNIE (membership no. Y66)
or
LEUNG YIM WAN IRENE (membership no. DZ127)
in the Proxy. You are advised to whatsapp the completed form to 9327 6216 or email to usrclbs@gmail.com as a record, in case the Club office comes across problems during their processing.
2) The completed form shall reach the Club's collection box on or before June 19, 2016 (Sun) 7pm by hand or by mail.
3) You may also hand in your completed form to one of our Subcommittee Members before the deadline.
